Anxiety and depression are the most common reasons UK patients seek medical cannabis prescriptions. Here is what the evidence shows and how to access treatment.

The Mental Health Crisis and the Limits of Conventional Treatment

Anxiety disorders and depression are the most prevalent mental health conditions in the UK. The NHS estimates that around 1 in 6 adults in England experience a common mental health problem such as anxiety or depression in any given week. Despite the availability of effective treatments — including CBT, SSRIs, SNRIs, and other antidepressants — a substantial proportion of patients do not achieve adequate symptom control with first-line therapies.

Treatment-resistant depression and chronic anxiety disorders represent a significant unmet clinical need. It is this group, in particular, that has driven growing interest in cannabis-based medicinal products as an alternative or adjunct treatment option.

What the UK Medical Cannabis Registry Shows

Data from the UK Medical Cannabis Registry (UKMCR) consistently shows that anxiety and depression are among the most common comorbidities reported by patients prescribed CBMPs, and that patients frequently report improvements in these symptoms following treatment initiation.

A 2025 registry analysis found that patients prescribed CBMPs reported statistically significant reductions in anxiety scores (measured using the GAD-7) and depression scores (measured using the PHQ-9) at follow-up. These improvements were observed across a range of primary diagnoses, suggesting that the anxiolytic and antidepressant effects of CBMPs may be a consistent secondary benefit of treatment.

How Cannabis May Help Anxiety and Depression

The endocannabinoid system plays a central role in regulating mood, stress responses, and emotional processing. CBD, in particular, has been extensively studied for its anxiolytic properties. It is thought to act on serotonin receptors (5-HT1A) and to modulate the activity of the amygdala — the brain region most associated with fear and anxiety responses.

THC has a more complex relationship with anxiety. At low doses, THC can have anxiolytic effects; at higher doses, it may paradoxically increase anxiety in some individuals. This is why most patients prescribed CBMPs for anxiety receive CBD-dominant formulations, or carefully titrated low-THC products under specialist supervision.

For depression, the picture is similarly nuanced. Cannabis may help by improving sleep quality (poor sleep is both a symptom and a driver of depression), reducing anxiety (a common comorbidity), and potentially modulating the neuroinflammatory pathways implicated in treatment-resistant depression.

Eligibility for a Medical Cannabis Prescription for Anxiety or Depression

Anxiety and depression can be prescribed for using CBMPs in the UK, but eligibility criteria are specific. Patients typically need to demonstrate that they have a confirmed diagnosis of an anxiety disorder or depressive disorder, and that they have tried at least two conventional treatments (such as an SSRI and a course of CBT) without achieving adequate symptom control.

It is important to note that CBMPs are not considered a first-line treatment for anxiety or depression in the UK. They are intended for patients with treatment-resistant or chronic conditions where conventional approaches have been insufficient.
