A Confusion That Matters
Walk into any health food shop in the UK and you will find shelves stocked with CBD oils, capsules, and gummies. Search online and you will find hundreds of brands marketing CBD products for everything from anxiety to arthritis. Meanwhile, specialist doctors are prescribing cannabis-based medicinal products to thousands of patients through private clinics.
Are these the same thing? The short answer is no — and understanding the difference is important for anyone considering either product.
What Is CBD Oil (the Health Shop Kind)?
CBD (cannabidiol) oil sold in UK health shops is classified as a food supplement, not a medicine. It is regulated by the Food Standards Agency (FSA), not the Medicines and Healthcare products Regulatory Agency (MHRA). This means it has not been through the rigorous clinical trials required for medicines, and manufacturers cannot make medicinal claims about it.
These products are typically derived from hemp (a variety of cannabis with very low THC content) and contain CBD as the primary active ingredient. By law, they must contain less than 1mg of THC per container. They are legal to buy and sell without a prescription.
The quality and consistency of CBD supplements varies enormously. A 2019 study found that a significant proportion of CBD products sold in the UK contained either more or less CBD than stated on the label, and some contained detectable levels of THC above the legal limit.
What Is Medical Cannabis (Prescribed)?
Medical cannabis — or cannabis-based medicinal products (CBMPs) — refers to pharmaceutical-grade products prescribed by a specialist doctor. These products are regulated as medicines by the MHRA and are subject to strict quality, safety, and manufacturing standards.
Prescribed CBMPs can contain significant quantities of both THC and CBD, in ratios that are precisely controlled and consistent between batches. They are available in several forms, including:
- Cannabis oils (taken sublingually)
- Dried flower (vaporised using a medical-grade vaporiser)
- Capsules
- Oromucosal sprays (such as Sativex)
The key difference from CBD supplements is that prescribed CBMPs contain clinically meaningful quantities of THC — the psychoactive cannabinoid that is responsible for many of the therapeutic effects observed in conditions such as chronic pain, PTSD, and spasticity.

If you have a chronic or treatment-resistant medical condition — such as chronic pain, PTSD, epilepsy, MS, anxiety, or a gastrointestinal condition — and conventional treatments have not provided adequate relief, a prescribed CBMP may be a more appropriate option. The clinical evidence for prescribed CBMPs is substantially stronger than for CBD supplements, and the products are of guaranteed pharmaceutical quality.
